SPOT "Self-Assessment" Submission Form

This is your Professional Self-Assessment response page. 

Use this form to answer the questionnaire for your Self-Assessment (see "Self-Assessment Questions" press button below).  All answers are multiple choice and require an answer from you.  Simply use the form below for the corresponding answer to each numbered question.  When you submit your answers, we will provide you with the results and what they might indicate. 

There are 46 questions and each has only two possible answer (either A or B answer for each question).  The purpose of this self-assessment is so that you might better understand what your preferences are currently (i.e., communication, interaction, etc.,).  This helps you be in a position for potential self-improvement.  And you also are able to better understand the preferences of others.  These are valuable qualities to gain as a professional. It promotes good teamwork on all levels.

This exercise is only a tool for improved professionalism.  Your honest response will determine the best indicators.
