SPOT "Self-Assessment" Submission Form

This is your Professional Self-Assessment response page. 

Use this form to answer the questionnaire for your Self-Assessment (see attached file).  All answers are multiple choice and require an answer from you.  Simply use the form below for the corresponding answer to each number question.  When you submit your answers, we will provide you with the results and what they mean.  In short, this analysis will simply show you what your preferred tendences currently are.  You will then have the opportunity to make an informed decision for improvement. 

There are 46 questions and each has only two possible answer (either A or B answers for each question).

This exercise is only a tool to help you to add value to your professionalism.  Your honest response will determine the best indicators and help guide you towards steps of improvement.
